What the form asks for

The cut sheet asks for your name, phone number, email, animal type, processing selections, and any notes you provide. Do not include payment-card information or other sensitive identifiers.

When you review or print

Preparing, reviewing, and printing a cut sheet does not itself submit an order. This site does not save cut-sheet drafts in your browser storage. Leaving or reloading the page can clear your entries.

When you submit online

If online submissions are enabled, submitting sends your information to the shop’s server for storage and review. A confirmation copy is queued for the shop and for the email address you enter. Email delivery may be delayed or fail; keep your on-screen reference.

The form uses a session cookie for security. The server also uses a hashed network address to limit repeated requests. Hosting providers may maintain their own access logs.
